The Town Clerk’s Office offers a variety of services. We provide official information between local government and the public. We are the “keeper” of Town Records. We also work closely with legislative bodies on recording and maintaining official records and documents. From reproducing birth, death and marriage certificates, to licensing your dog, to receiving the agendas and/or minutes of boards and commissions, to finding out who is running for election, the Town Clerk’s office is here to assist all in the most pleasant and helpful way possible.

Land Records
Recording Fees
The fee is $60.00 for the first page; $5.00 for each additional page for that document.
Nominee (MERS) fees are $159.00 for Releases and Assignments (when Nominee is the Releasor/Grantor), and $159.00 plus $5.00 for each additional page for other Nominee (MERS) recordings.
Conveyance Fees
There is a $2.00 conveyance fee for any deed with a consideration of $2,000.00 or more.
Local conveyance tax for the Town of Middlefield is 0.0025 x consideration paid.
No deed or permanent easement will be recorded without the State of Connecticut Real Estate Conveyance Tax Form (OP-236).
Map Fees
Recording site plans, easements, boundary maps, etc. are $10.00; a subdivision map (must be signed off by the P&Z Chairman) are $20.00.

This act required owners of foreclosed properties to maintain specified standards. This must either be registered with the Town Clerk or MERS. Form must be filled out and a fee of $100.00 must accompany it.

Copies of land records are $1.00 per page. Copies of maps are $1.00 per page; full size copies are $5.00.
Certified copies are $1.00 per page and a $2.00 fee for certification.

Vital Records - Births / Marriages / Deaths
MARRIAGE LICENSE
Marriage licenses must be obtained in the town where the marriage ceremony is taking place. There are no premarital blood tests required.
MARRIAGE LICENSES WILL BE ISSUED MONDAY THROUGH THURSDAY ONLY.
Listing of Local Justice of the Peace
At the Town Clerk’s Office, the bride and groom must appear together. The license will be prepared while the applicants wait and they will leave with their marriage license. (Allow up to 30 minutes to process the license.) The license is valid for 65 days from the date they come in to apply.
A “Marriage License Worksheet” (general information about each person, including their parents’ names/maiden names of their mothers, and the state or country of parents’ birth) will be filled out. It can also be brought in already completed.
A valid, government issued photographic identification (with date of birth, signature, and expiration date) is required.
The license fee is $50.00 (cash). If a certified copy is requested, it is an additional $20.00.
Copies
Certified copies of marriage certificates are available at the Town Clerk’s office in the town of occurrence, and in the towns the bride and groom resided in on the day of the marriage (Connecticut residents only).
The fee is $20.00 per copy. A valid, government issued photographic identification (with date of birth, signature, and expiration date) is also required.
If a request is made by mail, please enclose the form to “Request a Marriage Certificate”, a copy of the applicant’s valid government issued photographic identification, and payment in the form of a money order. Copies are usually mailed out the next day after receipt.

BIRTH CERTIFICATES
Birth Certificates are available for anyone whose mother was residing in Middlefield at the time of the birth. They are also available at the Town Clerk’s office in the town where the birth took place. Births in or after 2003 can be obtained from any Connecticut Town Clerk’s office.

You may request a birth certificate for yourself, your child, grandchild, parent, grandparent or spouse.
The fee is $20.00 per copy. A valid, government issued photographic identification (with date of birth, signature, and expiration date) is also required.

DEATH CERTIFICATES
Death Certificates are available for anyone who died in the Town of Middlefield, or who died in another Connecticut town, but was a resident of Middlefield at the time of their death.
Burial Permits are available to funeral homes for a fee of $5.00; Cremation Permits fee are $3.00. Sub-registrars are Biega Funeral Home and D’Angelo Funeral Home in Middletown.

New Dog Licenses
PUPPIES must be licensed within 30 days after turning 6 months old.
ADULT DOGS must be licensed with 30 days of obtaining them.
FEES – Licenses not within the 30 days are subject to a $1.00 per month penalty. The fee for a license is $19.00 for an unaltered dog, and $8.00 for a spayed or neutered dog.
RABIES – Rabies vaccination must be current and paperwork from a veterinarian must be provided.
SPAY/NEUTER – Documentation from a veterinarian must be provided.
REGISTERING – You may register in person at the Town Clerk’s office or by mail. If registering by mail, please enclose the “Dog Licensing Info & Form“, a check or money order, and applicable documents. All documents will be returned to you.
Renewals
JUNE is Dog License Month! All dog licenses must be renewed in the month of June each year. A postcard is sent out at the beginning of June as a reminder.
RABIES – Rabies vaccination must be current. If the rabies vaccination has expired since the previous year’s license, you must provide paperwork from a veterinarian of the new rabies expiration date.
SPAY/NEUTER – If the dog spayed or neutered since the previous year’s license you must provide documentation from your veterinarian.
FEES – The fee for renewing the license is $19.00 for an unaltered dog, and $8.00 for a spayed or neutered dog. Licenses not renewed by June 30th are subject to a $1.00 per month penalty.
REGISTERING – You may register in person at the Town Clerk’s office or by mail. If registering by mail, please enclose a check or money order, and applicable documents. All documents will be returned to you.
Transfers
If your dog is currently licensed in a Connecticut town and you move to Middlefield, you can transfer the dog’s license for a $0.50 fee.
If your dog is currently licensed in Connecticut and you become the new owner, you can transfer the dog’s license for a $1.00 fee.
Lost tags
If your dog loses its tag, a replacement can be obtained for $0.50

Notary Public Services
Notary Public Commissions – The Secretary of the State’s Office in Hartford issues Notary Public commissions.
Notary certificates from the Secretary of the State are recorded at the Town Clerk’s office in the town where you reside. The recording fee is $20.00.
You may also record your commission in the town where you do business, but this is not mandatory, and requires another $10.00 recording fee to that town. If a notary’s signature needs to be certified by the Town Clerk in the town where the commission is recorded, there is a $2.00 fee for the certification of signature.
Absentee Ballot Application
- Illness or physical disability.
- Absence from town during all election hours.
- Religious beliefs forbidding secular activity on election day.
- Serving as an election official at a polling place other than their own.
- Active service in the Armed Forces.
How to Apply – Applications for Absentee Ballots can be obtained from the Town Clerk, the Registrar of Voters, Democrat and Republican Headquarters, or at the Secretary of the State’s website.
Any qualified voter already out of town may have an application mailed to them. However, you must allow sufficient time for receiving and returning the application and the ballot.

Military Tax Exemptions
You must file by September 30th to receive credit on the following July’s tax bill. There is no fee to record this document.
Freedom of Information
Freedom of Information Requests Guidelines for The Public
- The Freedom of Information Act (Chapter 14 of the Connecticut General Statues) guarantees public access to public records that actually exist, and does not require that employees create new information that does not currently exist. Town employees always seek to answer questions from the public, but cannot be compelled to perform research or analysis to create information, which in form or content, is not already available.
- The public may be required to view requested information in a particular location in order to avoid loss of, or damage to public records, or to prevent disruption of normal work in the department.
